How do you enroll with South Country?
Medical Assistance programs provide health care coverage to Minnesotans with low to no income. Most Minnesotans can apply online through https://www.mnsure.org.
Some people should not apply through MNsure but will need to use a paper application. These are typically seniors age 65 and older or people with disabilities. The contacts below can help you if you need to use a paper application or have questions.
South Country is contracted with the State of Minnesota to provide the benefits of Medical Assistance programs to eligible residents in our service area.

Health insurance is complicated stuff. You don’t have to figure it out on your own.
- Connect with a navigator in your community to help you get enrolled. Navigators can help in several different languages.
- Call DHS Health Care Consumer Support at 651-297-3862 or 800-657-3672 for help with questions or for a paper application.
- Reach out to your local county or tribal office.
- If you have a disability, contact the Disability Hub MN at 866-333-2466.
- If you’re 65 or older, contact the Senior LinkAge Line at 800-333-2433.
Choosing South Country
When you are confirmed to be eligible, you will receive a letter from the Department of Human Services that shows health plan choices in the county where you live. If you live in our service area, South Country will appear on your letter as a health plan choice. When you respond to the letter and choose South Country, you will automatically be enrolled in the South Country program that fits your specific needs. What’s next?
Health care needs differ for everyone. If you are new to South Country and want to learn more about your benefits, you can call Member Services to get answers to a wide variety of questions. You can also ask us to mail you a paper copy of your member handbook, provider and pharmacy directory, or drug list at no cost to you.
Depending on your program, someone from South Country will reach out to you after you join. You will also start receiving information from us, including a South Country member ID card to use when you get care. It is important we have your current contact information so we can stay connected. We are here to help you stay as healthy as you can be.
