Medicare Part D: The Medicare prescription drug benefit program. We call this program “Part D” for short. Medicare Part D covers outpatient prescription drugs, vaccines, and some supplies not covered by Medicare Part A or Medicare Part B or Medical Assistance. Our plan includes Medicare Part D.Medicare Part A: The Medicare program that covers most medically necessary hospital, skilled nursing facility, home health, and hospice care.
Medicare Part B: The Medicare program that covers services (such as lab tests, surgeries, and doctor visits) and supplies (such as wheelchairs and walkers) that are medically necessary to treat a disease or condition. Medicare Part B also covers many preventive and screening services.
Medical Assistance: This is the name of Minnesota’s Medicaid program. Medical Assistance is run by the state and is paid for by the state and the federal government. It helps people with limited incomes and resources pay for long-term services and supports and medical costs.
It covers extra services and some drugs not covered by Medicare. Medicaid programs vary from state to state, but most health care costs are covered if you qualify for both Medicare and Medicaid.

We are delighted to welcome you as a new member of South Country! As part of our plan you have access to a dedicated network of health care providers and a team committed to supporting your health and well-being.

Our goal is to make your transition as smooth as possible. There are many resources available to you, with some of them listed on this page. We encourage you to reach out to Member Services with any questions about your coverage, benefits, or available resources.

You will be receiving new enrollee information by mail. This includes your health plan ID card you will use to get health care. Some members will get a new enrollee screening in the mail, and others will be contacted by a care coordinator. These steps help us connect you to the care and services you need. If you receive a screening form, please take a few minutes to complete it.

South Country was created to do what traditional health plans and county agencies couldn’t do alone. We bring medical care and social support closer together, close gaps and helped our members truly get healthier. We are passionate about doing good things to help the culture and attitude of South Country. It comes from being part of the local rural community. It inspires us to go above and beyond to serve people who need extra help.

Our local representatives, called connectors, live and work in the rural communities of our service area. They provide in-person support for our members who may have trouble navigating the health care and social service systems.

Connectors are part of what makes South Country a “health multiplier.” We multiply the impact of care in rural medical care, public health and social ways that most health insurers cannot. Through closer relationships with local providers and social support networks, we integrate care in communities and close the gaps.
